Please complete and submit this form by 5 pm AWST, Friday, 2 January 2026 to register your band's participation in the Anzac Day March through St Georges Terrace (Perth) on 25 April 2026. If this form has been sent to the wrong person please contact events@rslwa.org.au. The order of march and form up details will be communicated in March 2026.

This is a digital form and must be submitted online so that we collect all the information to plan the Anzac Day March. Please do not attempt to print and complete this form by hand.
